Child Safety and Permission to Secure Treatment

Child safety is very important to Yak Academy.  Our curriculum is designed with safety in mind and most of our teachers and staff are trained in CPR.  We expect all parents and children to follow safety instructions designed to enhance the safety of all participants.  In the event of an emergency related to your care or your child’s care, I authorize Yak Academy to secure medical treatment from any licensed physician, hospital, and/or medical personnel.  I agree that I will be responsible for payment of any and all medical services required.

Day Care Center Status

While Yak Academy follows safety policies consistent with a daycare center; Yak Academy is not a daycare center.  Therefore, the parent or caregiver of the child must be immediately available while his or her child is in class or on the Yak Academy premises.

Insurance Coverage 

While Yak Academy carries general liability insurance for business operations; it does not carry medical insurance for injuries sustained in one of our programs.  Therefore, we advise parents to review their own health insurance policy for coverage.

Tuition Plan Details

Yak Academy Learning Centers

 

Monthly Payment Plan:  The first payment is due upon registration and future payments are charged automatically on the first of every month.  A credit card must be used for this payment option.

 

Semi-Annual Payment Plan (Plan option available until October 12^th):  Save 10% if you pre-pay the school year in 2 payments.  The first payment is due upon registration and the second will be charged in February.  Although you may pay by check, a credit card must be on file for this payment option.

 

Annual Payment Plan (Plan option available until October 12^th):  Save 15% if you pre-pay the entire school year.  Payment is due upon registration.

 

Mobile Program Location:  Payment Plans and discounts are site-specific.  Please refer to your location's schedule for details.

Cancellation Policy

Yak Academy Learning Centers

 

Monthly Payment Plan Cancellation*:  A 30‐day notice is required prior to the next scheduled payment.  Full month tuition is required for final month. 

 

Semi-Annual Payment Plan Cancellation*:  A 30-day notice is required prior to the next scheduled payment.  We will refund the remaining prepaid tuition if there are unused classes minus a $100/child cancellation fee. 

 

Annual Payment Plan Cancellation*:  We will refund the remaining prepaid tuition if there are unused classes minus a $100/child cancellation fee. 

 

*No refunds or credits are available for missed classes; however, if you are unable to attend a class, you may reschedule your child for any class that has availability throughout the school year. Must be a current customer. 

 

Mobile Program Location:  Fees are non‐refundable.  Refunds, makeup classes or attendance to a different class for participants is not permitted unless Yak Academy or the facility at which the class is hosted cancels a class.

Bathroom Policy

Yak Academy Learning Centers:  In the event that your child needs to use the bathroom, a teacher or Director will direct the child to the bathroom. We will help a child who asks, unless otherwise instructed by parents.

 

Mobile Program Location:  In the event that your child needs to use the bathroom, we will abide by the policy set‐forth by the facility at which we’re hosting the classes. If there is no policy, then a teacher or Director will direct the child to the bathroom. We will help a child who asks, unless otherwise instructed by parents.

Waiver and Release

While I understand that Yak Academy is concerned about the safety of my child, I also understand that the Yak Academy activities for which my child is enrolled may be of a strenuous, hazardous, or physical nature.  Knowing the risk involved, nevertheless I voluntarily request permission for myself or my child to participate in the described activity.  I also understand that Yak Academy does not accept responsibility for injuries, damages or loss that my child may suffer while participating in Yak Academy programs. Accordingly, I assume the full risk of any physical injuries, damages or loss, regardless of severity, which I, my spouse or caregiver, or my child may sustain as a result of participating in any and all activities connected with or associated with any Yak Academy program.  On behalf of myself, my spouse, my child and any caregiver acting on behalf of myself or my spouse, I hereby waive, relinquish, fully release and discharge, and further agree to indemnify, hold harmless and defend Yak Academy, its owners, officers, directors, agents, contractors, employees and landlords against any and all rights, claims, causes of action and liabilities of any sort that I, my spouse or caregiver, or my child may have now or in the future, including but not limited to, any claims for personal injuries, medical expenses, property damage, or losses sustained by me, my spouse or caregiver or my child arising out of or in any way associated with the activities of any Yak Academy programs.

 

I certify that I have custody or am the legal guardian of said minors by court order, and that I and my minor children are physically able to participate in the described activities.

 

I have read and fully understand the sections on Child Safety and Permission to Secure Treatment, Day Care Center Status, Insurance Coverage, Bathroom Policy and Waiver and Release.

 

I also understand the Tuition Plan Details and Cancellation Policy and agree to such terms.
